Camper Liability Acknowledgement Form

    Camp Dubonett - 10105 Lake Dubonett Trl, Traverse City, MI 49685 - 231-486-5291 - CampDubonett@gmail.com

Thanks for staying with Camp Dubonett. We truly appreciate you giving us the opportunity to help you experience the Traverse City area and the great outdoors and all of the other amazing things about this area. Because you are choosing to make part of your experience staying outside and enjoying activities in the woods, we unfortunately need to make you aware and ask you to release us from liability from some of the hazards and risks.

The following are some, but not all, of the conditions that might result in injury, harm, or death at Camp Dubonett:
    • Participating in campground activities involves known and unanticipated risks which could result
in physical or emotional injury, paralysis or permanent disability, death, and property damage. Risks include, but
    are not limited to, broken bones, torn ligaments or other injuries as a result of falls; burns from campfires;
drowning in pools or other bodies of water; falls from play equipment or caused by uneven surfaces; medical
conditions resulting from physical activity; and damaged clothing or other property. (These risks simply
cannot be eliminated, despite the use of safety equipment, without jeopardizing the essential qualities of the activity.)
    • There are many foot faults that can trip or injury someone.
    • There are branches and limbs that fall out of trees which can be tripped over or land on someone or something.
    • There may, periodically, be wild animals in the woods that may approach your camp, destroy items or attack humans and other animals.
    • You and your guests may provide faulty equipment, fail to make a reasonable determination about individuals’ ability, fail to warn of specific hazards on the premises or intentionally not warn your group to adhere to safe practices.
    • There are open fires near forest and farm areas that contain many combustible objects. Fires are always a risk to everyone’s safety and must always be monitored and completely extinguished before leaving it.
    • Being a passenger on an animal or spectator at animal activities.
    • The unpredictability of animals to react to sounds or sudden movements, unfamiliar objects, persons, or other animals, a collision with another animal, people, or an object.
    • Hazards, including, but not limited to, surface or subsurface conditions. (slippery cement/asphalt)

I represent that I have adequate insurance to cover any injury or damage I may suffer or cause while participating in
this activity, or else I agree to bear the costs of such injury or damage myself. I further represent that I have no
medical or physical condition which could interfere with my safety in this activity, or else I am willing to assume
and bear the costs of — all risks that may be created, directly or indirectly, by any such condition.
